I'll admit to not having any doctors in my circle of friends. I do have some relatives who are on the dole. I never heard of them getting any free samples. Never heard of them asking and not getting them, either. So I have nothing much to base my observations on, just my current GP and what I see in her office: lots of feeble old people. And I've seen a pharma rep there three times, which is half my visits to her to date. My former GP was a boutique doctor. I was with her for a couple of decades. She didn't accept any insurance let alone take indigent patients. I never saw any sign of her holding or dispensing free pills.
