Medicaid is designed for people of all ages. Medicare is designed only for old people. I'm not sure that Medicare has simpler bookkeeping than Medicaid. If Medicaid is more complex, its probably because of the attempt to limit it by income, but if everyone can get it, than you wouldn't have this complication.

Not that I would advocate Medicaid for everyone, I just don't see how Medicare for everyone would be better, easier or cheaper.

Medicare probably provides better coverage now, but its also more expensive per person. Expand either to cover everyone (or merge the two and have everyone covered) and you change the political dynamic behind the lower funding for Medicare.
