Mathematically, how long will it be before everybody can receive the same care.

If by the same, you mean equal to what middle class Americans get now, then it should be achievable eventually.

But in the meantime the level of care will go up, and the level of care equal to what we get now will seem substandard.

If by the same, you mean everyone gets exactly the same care. Well I don't really see that as a goal, or something that can be achieved without unjustly lowering the quality of care for the wealthy.

If by the same, you mean how long before everyone in the US can get care that for the time is as good as what you or Lane3 now get? Well the trends are working against you not for you. Health care costs are growing faster than the economy. Of course they can't do that forever, but they can do so for some time, and even after they settle down a bit they might roughly keep pace with economic growth. So its not likely that you can grow yourself in to everyone being able to get abundant high quality care.
