I think the first priority about health insurance is getting it to the uninsured.

Chickens and eggs. The first priority is to get the cost down through structural changes. Then there will be fewer uninsured to deal with. There are uninsured and then there are uninsured. Most of them can be dealt with through cost reductions. Many more through structural changes. If you deal with the uninsured first, demand goes up and so do costs. Much tougher and much less likely to get the structural changes done later.
