We are the richest country in the world, of course we pay more for health care.

Are you familiar with the difference between % of GDP cost and absolute cost?

When I said we spend twice as much as other advanced countries I specifically said as a percent of GDP. That removes relative wealth as a factor. In absolute numbers we are spending more than twice as much as other countries.

So, if you look at our costs as a percent of GDP, our health care is half as efficient as other advanced countries'. And if you look at large population health outcomes, which, in our case, are consistently near the bottom of the countries in the OECD, it's even worse than that.
