I have yet to hear anybody give a good reason why companies would silently pay billions to GSM IP holders but complain constantly about smaller payments to Q.

One possibility is that Qualcomm is a big target. Even if the Koreans & Chinese handset manufacturers are paying GSM royalties, it's likely to be a small amount to many patent holders. 5% to a single entity is simply more obvious than getting nickel & dimed (death by a thousand cuts, although "death" is probably the wrong word considering the margins that the asians make on GSM.).
