A federal alternative doesn't amount to total deregulation, not even close. And it would probably lower costs (unless the federal regs are so expensive that the companies keep the "state charters" to avoid them)

Allowing them to "pay claims when they feel like it" (and so presumably not pay them when they don't feel like it) would go beyond even extreme deregulation. It would be allowing fraud.

The countries in the Frontline piece do it by more regulation

More regulation overall? Probably. More regulation of health care insurance? I'm not so sure about that.
