No, it's based on the income tax filing for the year before. My current Medicare premium is based on my '06 federal tax return. Next year's premium will be based on my '07 tax return.

For first year retirees, I guess it would be based on their pre-retirement income, whatever income they reported in '06. That would make a large proportion of first year retirees "wealthy." <g>
