I'd refuse to go on O'Reilly's show, too. The man likes to shout his guests down. It's rude. My guess is that Hillary negotiated a special agreement with him to get him to behave.

As for the debate question, your interpretation is one way of looking at it. My feeling is that he very reasonably felt that the entire exercise was pointless, and that it was demeaning to everyone involved, and he decided there was no advantage in risking involving himself in something like that again.
