In the one case, I was receiving radiation treatments at Cedars Sinai. Cedars, of course, is an in-network hospital. But apparently, my radiation oncologist is part of a medical group that is not in network. I had no idea that was the case. I never saw him outside of Cedars, and it never occurred to me to ask if his services were covered in network.

There is an out of network, out of pocket maximum, but it does not apply to any amounts considered to be above the customary fee. I didn't know that, but there it is. Such exclusions may also be a part of your policy, whether you know it or not.
