My first guess, which will probably never be confirmed one way or the other, is that He had never been checked for heart disease. It's so easy these days to find the kind of problem that Russert probably had, and treat it.

I think he was aware of a problem. There was a report he had a stress test in April. Wouldn't be doing that unless there was a question about his heart function.
