i found they went by zip codes to determine customary fees. you can see fees range from large hospitals in philly to those along the main line and then those further out.. so if you get a doctor who is associated with the city but works out of two locations and charges center city prices you may find the insurance company come back and limit payment. it happened to me back in 80's. i laughed at them and told them they will pay me one way or another. i will take it the state attorney generals office if their complaint system doesn't satisfy me. i quickly got a reply back in a couple of weeks that charge had been accepted.
