As far as I'm aware, it was a Zuni rocket from another aircraft which was accidentally fired at McCain's aircraft. I don't think McCain is to blame for much more than being exceptionally lucky in surviving his accidents. I'm rather more sceptical of his plan for disappearing the Federal deficit.
