I had a midwife and no episiotomies with any of my kids. I had a little tiny tear with the first baby (3 stiches) and no tears with any of the others. Perineal massage during the birth helps a lot to stretch you properly. I am totally against episiotomies except in the rare case of a women whose skin is not going to be elastic enough to stretch properly. Also, no drugs. Much better control without them, and the only part that REALLY hurts is crowning, and by then you're almost finished.
