AFFIDAVIT OF GEORGE W. BUTLER, PRESIDENT AND CEO,
FIRST NATIONAL BANK OF BARNESVILLE
I, George W. Butler, first being duly sworn and deposed, state as follows:
1. I am George W. Butler, the President and CEO of First National Bank of Barnesville ("Bank"), a Georgia corporation.
2. The Bank has an outstanding debt owed to it by Plasticon International, Inc. in the amount of $1,336,385.59.
3. Plasticon International, Inc. has made payments on the debt described in the preceding paragraph. The sum of these payments is $111,654.36.
4. All of the payments described in the preceding paragraph were erroneously applied to James N. Turek's account with the Bank. This misapplication of funds was earlier corrected and all payments made on the debt have been applied to the Plasticon International, Inc. debt, which results in the balance outstanding as provided herein at paragraph 2.
5. For at least the past five years, the Bank has received no payments from Plasticon International, Inc. other than those previously described as received and having been erroneously applied to the James N. Turek account.
