I wasn't clear in what I was trying to depict.

Let's try again:

Got a bill summary from insurance co.

Item, billed, covered, ins. paid, patient responsibility

1. billed $100, $100, $30, $0
2. billed $120, $120, $25, $0
3. billed $170, $0, $0, $170.

Item 1 and 2. are 'covered' and patient pays nothing and
insurance paid the provided $30, $25.

Item 3 is 'not covered', patient is responsible to pay for
$170, with no noted discount and will be dependent on the
hackling between provider and patient.
