Well they can leave their card at home, but they still should be able to not be members if they don't want to be.
Also if there are real practical concerns, not just "I don't want to join". From the two links I posted

They probably pay more for health care than that, but they might go to doctors who don't take Medicare patients, so Medicare might not help them any, and it is a cost. |
