I think when they say belly fat, they mean visceral fat, inside the abdominal cavity, not the under the skin (subcutaneous) fat.

I don't know much about liposuction, but I am under the impression that they don't do visceral fat.

My other comment on this would be that yes, visceral fat itself is not just dead weight, it is an active organ, making things worse, but that is not the only problem. The problem is a lifestyle that leads to visceral fat accumulation.

As far as which component (accumulated fat or lifestyle that leads to fat accumulation) is responsible for what percentage of the effect LB linked (Doubling of death rate) - I have no idea.
