Interesting that more than a few groups did due diligence on Madoff, saw things they did not like, and declined to invest or took their investors out.

So it may be that a large number of people he approached declined to invest.

I think what made it work for so long : He selected victims who had lots of money, and so very few would need to take their money out.

So the scheme is primitive, the target selection and recruitment precise.
