There are no such rules - not in this country, anyway. The patients and families are always part of the process. As far as Hospitalists are concerned, think of them the same way you always thought about the traditional "Attending Physician" - for that is exactly who they are for the duration of the admission. You should expect - and demand - the same degree of care and attention. For the Hospitalist it may not be easy to step into the shoes of someone whom the person may have known for many years - but that is an integral part of the job.

In my group in order for us to accept a patient their Doc must call one of us and discuss the case directly.

It is my understanding that you were under pressure to go home before you felt it was the right time. If that's the case - yes, that may happen because of pressures from the government. The moment their reviewers feel that the admission no longer fulfills their "acute" criteria, they start cutting reimbursement to the hospitals - and they are quite merciless. They are a pain to deal with, and "appeals" are usually useless.

I've considered becoming more outspoken about all this, maybe write... maybe I will... but the impact that one guy (without bureaucratic leverage) can have... ain't great... :)
