Hi Tom,

You are welcome. One other point I forgot last night... Plex is not without its problems.

The recent PML related death after PLEX is likely more IRIS related [Immune Reconstitution Inflammation Syndrome]. So, with a few more datapoints, the Plex cycle might be optimized to remove as quickly as possible without triggering IRIS.

THe following article gives some background on IRIS and its likely triggers: aidsmap.com However AIDS JCV and PML may not be an accurate model for MS/Ty/immune system treatmenets results.

So the rough numbers [about as accurate as one pass of the dice at a crap table] are:

Between 1 in 1000 and 1 in 6000 PML at 17 months
About 25% risk of IRIS after PLEX
Maybe death rate from PML [including IRIS] could still get to about 1 in 10K.

This would be a lot easier for patients and Drs. to look at than 1 per thousand. I doubt the data will ever support risk of death at 1 in 100K, so PML remains on the label [and it is black].

Life is never dull on the ELN roller-coaster,
graham
