The size of the pool doesn't matter much except for big ticket items like heart transplants where one incident can throw premiums out of whack.

Whether you have a nation-sized pool or a company-sized pool, either you set the same premium for everyone regardless of age and overcharge the young people or you charge more for older people, who cost the most. You can make a case for doing it either way.

Between ages 55 and 65 is the most costly range for private insurance. Before that you don't have a lot of medical bills. After that, Medicare pays them. In that range, they'd rather not have your business. I've talked to people who couldn't get any just based on age. So, to insure you, they expect to be paid a premium.
