"I know that some people hold that view but I've never heard anyone offer a cohesive rationale for it nor can I imagine one."

I believe I've heard the current President of the United States holds that view... Of course, our last one said 'you can get healthcare by going to the emergency room', so we are getting there.

Instead of posting endless articles from the web supporting my view, I'll simply say that when you have cases where citizens die or lose everything thru illness or lack of insurance, the government should step in with a system preventing that. The current system is a competitive roadblock for business in the United States, it's overly expensive, and doesn't provide the kind of preventative care we need. If in a new system there is less 'profit' and surgeons have to settle for making less than a million a year, so be it. IBM is currently telling it's employees that will shortly be laid off - 'we have positions in places like India that you can take at Indian wages if you like'...
