Why? That's easy: there is no profit motive.

I would point more toward the mindless standardization that comes with big and bureaucratic and the irrationality that comes with politics. I don't think that the profit motive is essential to quality. (I'm a retired Fed. <g>) But monopolies, particularly monopolies guided by politics, are killers.

The patient can't pay for it. That's what insurance is for, right?

The doctor in your example was being creative. We still have creativity in our system. I suspect that those who are fond of their socialized systems, like the Canadians, are victims of the lack of creativity. The doctors follow the standards. The patients never know the difference because all their neighbors are treated to the same standards so everyone feels like it's working well. The environment contains no outliers so there is no feedback to disrupt their sense of well-being.
