IF this country could figure a way to lead the world in preventive health care, we would probably have the most effective and cost effective medical system in the world.

I'm not so sure about cost-effective and therein lies the rub. I think that it's cheaper to let a diabetic die young even with expensive end-of-life treatment than to pay for the massive amount of low and moderate cost treatment over an additional twenty or thirty year lifetime. The simplest and most direct way to make the system less expensive is for more people to die in middle age.
