OT/ Politically, I don't think we can have single payer first and then work out exceptions later. The freedom needs to be built in from day one. Many people pushing for health care reform will want the extreme cost containment from single payer - the old Hillary Care would have made it a FELONY to pay your own doctor.

So I expect that the ability of individuals to pay for their own medical services and be able to control will need to be in the plans from day one.

People already hate the idea of insurance companies reviewing their treatment plans. Government review will be about as popular as the TSA.

There are roughly 45-55 million uninsured people in the US out of a population of 300 million.

Otherwise, the 20-30% of the people with good insurance and/or enough money to pay for what they want will just kill this politically. I expect that 99% of the people who make campaign donations are happy with their access to medical care.

The French System sound pretty good, except they don't pay their doctors enough.

This would be a problem in the US, because many bright people who would be in say neurology are going to end up in law school, wall street, or technology. IF we pay docotors less money then society is saying they are contributing less value.
Only the military and the clergy can have low pay and high status.

Anyone else with low pay who thinks they have high status is regarded as a schmuck.

There has already been a huge shift in preference from med school to law school.

Doctors already in practice might spend more time on real estate or a small business.

In the US, money talks.
