But before even going there, you have to first conclude that a person can receive health care in Costa Rica (or wherever) that is as good as that received in America.

No, you don't. The person buying the health care needs a comfort level with his transaction or he won't proceed. What anyone else thinks about someone else's decision is irrelevant. And, in any case, the quality of care in Costa Rica has no impact whatsoever on the quality of care of someone using a US facility gets.

That said, the reduction in facilities will NOT be in proportion to the reduction in patients, even over large areas of the country.

It will over time. I've already acknowledge that there is some disruption during transition but then change is always a bitch that way.

So, suppose enough people choose to go to Costa Rica for their heart surgeries from this area such that they can no longer perform ANY heart surgery there. The town currently has several cardiac surgeons. What now?

Over time, people locate where there are medical facilities and medical facilities locate where there are people. Towns dry up all the time and people make their choices about living in them or leaving them. We lost a lot of towns to the Interstate Highway System. That sure didn't mean that the quality of transportation went down.

The bottom line is that when you pull money out of the American health care system -- ... -- you will generally degrade the quality of the service for everyone.

Your continue to reassert that belief without demonstrating evidence or logic to get to that conclusion. Your belief doesn't make it either a fact or a valid conclusion.
