Well, supplies and equipment are separate items.

But the RVU scale is all-inclusive -- it takes into account time, effort, technical ability, judgment, stress, and cost of the physician's education, as well as various practice operating expenses like malpractice insurance cost as well as the location (a given procedure in Arkansas will be a lot less expensive than the same procedure in NYC).

It actually is a fairly decent metric. I did a lot of work with it about 10 years ago -- we developed a cost accounting and contract analysis system for physician practices around the concept that actually worked reasonably well.
