ensure physicians will have a tight rein on the activities of well-trained nurses.

I see no problems with that. How are those nurses supposed to become "well trained" if they would not have physician supervision?

In an actual working relationship, and when the nurse is really well trained, the physician supervision takes progressively less time. Both the doc AND the nurse soon enough learn to recognize the occasions when they "need to talk".
