picture of paranoia.4,5 Objective features include
anger, irritability,attention to small details, guardedness, evasiveness, litigiousness,hostility, humorlessness, self-righteousness,
hypersensitivity, sullenness, and suspiciousness.

The absence of deterioration and other psychopathologic symptoms distinguishes delusional disorder from other psychotic disorders. However, diagnosis is complicated by the fact that these patients do not initially seek psychiatric help. Instead,they often focus on the delusion or mood. In effect, psychiatric illness is denied.
Furthermore, their mental state is surprisingly normal; thinking,
orientation, attention, memory, emotional responses and perception, are intact. Responses such as hostility, anger, sarcasm,
and lack of cooperation commonly seen in other paranoid conditions
may also be present. Since these patients are mentally intact aside from the delusions, they are capable of acting on delusions unencumbered by confusion, hallucinosis, or major motor symptoms.

In mania, paranoid features may arise that are associated with heightened mood, excessive self-confidence, and grandiosity.
