Peter B. Bach, a pulmonary specialist at Memorial Sloan-Kettering Cancer Center, was a senior adviser to the administrator of the Centers for Medicare and Medicaid Services from 2005 to 2006.

So, I wonder whether he'll move from Sloan-Kettering to Jefferson Memorial Hospital in Pine Bluff, Arkansas? There, the hospital has to pay through the nose to have specialists move into town to serve its population of about 100K (including the county).

I've watched them for 15 years. Specialists move in, take the 3 or 5 year deal the hospital offers, and as soon as the deal is up they go, "Oh, hell. THAT was a mistake". And they're gone.

This is really something that the markets need to deal with as best they can. There are already local adjustments in Medicare fees to account for regional pricing. If you start doing too much social engineering the quality of health care is going to suffer. We don't need the government telling physicians where to live.
