has it controlled costs or mandated what they will pay for services and private insurance has picked up the difference and passed through these costs in higher insurance premiums.?

Excluded from medicare costs are the increases in processing documentation to process billing. All i hear from care givers is "its a nightmare". Who is paying for the extra overhead to process all this additional paper work? We are in with higher insurance costs because medical services need to increase prices to cover their overhead... costs they cannot pass through to medicare.
