It is true that most patients are not in Medicare HMO's. However, the entire Medicare program is administered through local intermediaries - "carriers" - who are private insurance companies. I'm told that those carriers charge Medicare only their "cost" - but in reality it is not at all clear what those costs are. I was wondering that perhaps before medicare gets expanded to many more millions of people, it would make sense to eliminate those local carriers - and try running the entire program as a single payer program. That would generate important experience.
