I think doctors and hospitals would prefer to work without insurance (or at least through a single billing system). My local hospital had several RNs that ONLY did paperwork associated with insurance companies. When my wife started working the paperwork quadrupled as the insurance companies argued over every matter and treatment plan, pointing fingers at us and not at one another. I think it was some sort of "code" like the mafia uses. Unless there is an insurance fraud suspected (which is a police matter) I don't see why we need any insurance company at all. Take all the premiums people are spending now, and put those into a trust account. We'll work on creating unified and normalized deductibles and co-pays later based upon means and taxable income.

One caveat - if a hospital has done a procedure write down in the past, that is the price of the procedure, not what they charged some combination of payers and agencies, or used as tax-dodge retail write offs for taking care someone who was uninsured.

The people who are least able to pay are charged the most because this "uncollected amount" is written off against earnings, even though they have performed the same procedure for frequently half this amount. Let's put an end to the accounting games.
