I don't disagree. My father was a GP and surgeon, as were my grandfather and uncle. People paid mostly out of pocket for the treatment in those days (60's & 70's). They did great, not ultra rich but comfortably wealthy. But then, in those days there was a lot less expensive technology.

Sometimes i think the government should be the source of catastrophic coverage... maybe situation where the costs for an 'incident' rise above $20K. Then let the private insurance companies offer plans that cover costs under those amounts, if people want to buy in. If you take the $20K+ risk away from the insurers they could probably lower their prices dramatically. And up to $20K won't drive many people into bankruptcy if they are uninsured.

There really are no pat answers.
