A lot of money goes into R&D, trials etc. More drugs fail than are approved. Without incentive to regain expenditures and some profit and the future of drugs are doomed.

You must also take into consideration that drugs compete with other drugs for every application and just as there may be no competition upon approval a new drug from a competitor can wipe out any hopes of profit or severely limit it. Than there are law suits etc...etc etc
