Yes there is a co-pay, my experience has been that it is fixed depending upon your insurance company.

There are also the deductible amounts that often must be met before the insurance pays. Once the deductible is met depending on the policy other figures kick in based upon the policy.

Yes, have seen all of this----I have been the manager for my family on all of this.

That is one reason why I believe that the American people need to have a forum, a national conference at all levels local, state, national levels to talk about what doesn't work and what does work.

Obama and Congress are just one part of the pie as well as the insurance companies being one part-----the other big part are the people.
