That's a different kind of "group." That's a membership group, not an identity group, for example, "families with small children."

Of course HMO's are cheaper than PPOs. Their business model is entirely different. HMO's are designed to be cheaper. They are more efficient because they are centrally managed, their physicians are required to conform to efficiency measures and to stay within treatment limitations (rationing), and the physicians are salaried.
