People don't automatically enter the process of dying at age 68, when these advance care consultations are to start.

You're talking about a directive made when one is already in the process of dying. No one in reasonably good health and who is sensible would at age 68 sign away their right to ever recieve antibiotics, nutrition or hydration therapy.
