You don't sign away any rights to receive anything when you sign an advanced directive. The advanced directive only comes into effect if you are incapacitated and unable to communicate. It is wise to also appoint an attorney for personal care and give him or her a copy of the directive. That person is then able to instruct care givers on your behalf. Sometimes, in an emergency patients receive treatment that they may not have wanted.
