Whoa. That depends entirely on what the directive says.

You don't sign away any rights to receive anything when you sign an advanced directive. The advanced directive only comes into effect if you are incapacitated and unable to communicate.

A clause that says restrictions only come into effect in extreme circumstances would be wise.

BTW incapacitated and unable to communicate shouldn't include being uconscious after a spill down the stairs, or having a traffic accident, etc.
