You would be more likely to have doctors concerning themselves with the health of their patients as distinct from practicing medical entrepreneurship. There would still be needless procedures, but it would likely tame the current epidemic of them. It would also dampen the enthusiasm to pursue the profession among those who mainly see medicine as an opportunity to be wealthy. Doctors could continue to pursue wealth by developing new technology to which they can claim intellectual property rights -- this would address part of your concern about the need to push the envelope of care towards more difficult cases.
