The Doctors I attend in Beverly Hills accept both Medicare and Medi-Cal although they prefer Medi-Cal to Medicare if given a choice. But they only accept PPO plans, not HMO.

They perhaps have a lower percentage of Medi-Cal and Medicare patients than they have Blue Shield type PPO plans in less affluent communities -- but, remember, it was precisely this Blue Shield PPO reimbursement rate that you found so impossibly low.

Uninsured patients must pay 50% when services are rendered, which is likely just about the highest reimbursement rate these Doctors receive. Perhaps Doctors in Beverly Hills have a larger percentage of uninsured patients.
