Thanks MJ and TideGlider for that info. Each State seems to have a slightly different form that was used at different times. In the "old days" before 1915 or so, depending on the locale, births were recorded by the Town Clerk. Now the hospital or Doc has them filed in the County of record.

One of our children was born in Buffalo, NY in the 60's. The Vital Statistic lady finally got mad at my husband and me, because we couldn't decide on the name for this child. She threatened to keep me in the hospital until I had the info for the form so it could be recorded!
