I googled the subject, but interestingly, it seems hard to find out how it all came about. Physicians - and their for the most part irrelevant organisations - do not make laws. Hard to figure out. I did find, however, this book sized text published by the government a few days ago, containing rigid set fees for just about everything under the moon. For many, many years now - advertise or not - those are the guys who tell you exactly what costs how much.
