It's definitely not as radical as some make it out to be. Not even close. But it is in many important ways a move in the wrong direction that could well end up being radically and critically so. If one wants to evaluate directions, it is useful to extend them to their extreme as a thought experiment. That makes it easier to see that the direction is wrong. But claiming that the first steps in that direction are radically wrong is either intellectually dishonest or intentionally hyperbolic.

As for PhARMA and AMA's support. There are other possible explanations. It could be that they don't really represent their constituencies. That happens a lot. An ideological cadre of lesser lights hijacks a professional association, for example. An addendum to the "those who can't do, teach" is "or run the professional association." I have had a lot of experience dealing with union leaders who are either thick as a brick or more interested in the interests of the union establishment than the interests of their members. Another explanation is that they are resigned to a change they don't agree with but are going along to try to get the most they can out of a bad deal rather than being shut out entirely. Or that their leaders are full of themselves and hunger to be invited to the elite power parties. I don't think we can know what their support means so I don't put much stock in it.
