These are really painful discussions to have because it's like going back to epi 101. I'll put the pieces together for you, but I'd suggest you pick up a book on public health or epidemiology and explore it yourself.

Alcohol related injuries are among the top 5 cause of morbidity and mortality in the country and by extension, huge burdens on the health care system.

Our de facto system way of addressing this is to ignore it. Why? Health insurance does not incentivize or even compensate nurses/physicians for doing preventative work. Insurance companies pay for procedures first, treatment second and sometimes begrudgingly, and public health measures almost not at all.

The end result is a misallocation of resources that is one of the reasons why the US has high rates of acciental injury and higher rates of er usage than other countries. This is despite the fact the US was the pioneer in the research demonstrating that injuries related to alcohol misuse could decrease through preventetive interventions.

Read the article I posted and then ask yourself "why aren't we doing this?" and you'll have some insight into why the current system is failing.
