Dave, I can guarantee you that a statistical analysis comparing "gateway" clerk's raises, promotions and firings with the number of claims they denied, people they dropped and cheaper courses of treatment they instructed doctors to take would show a VERY strong positive correlation. (But, no health insurance company would ever ALLOW such a study! ) There will only be a paper trail if the managers were stupid. It's how the insurance companies make more money every quarter, earning their management obscene bonues. It's not just criminal - it's SINFUL.
