Seems to me that halting employer-provider health care and compensating the employees for that loss with the equivalent dollars in salary for buying their own insurance increases rather than reduces competition and increases consumer choice. Employers are for the most part only a filter between the patient and the insurance company, not an insurance provider.

Why not make it fully taxable and end the tax preference that distorts the market for health services?

The distortion that I think you're referring to was already ended in his #1, which eliminated employer-provided health care.

7. What is an insurance exchange?

I believe that that's a venue where insurance companies list their offerings so that the customer has one-stop shopping. I don't know that that's an essential feature but customers tend to be confused about insurance so anything that makes shopping for it and understanding what they're buying easier would seem to me to be useful and non-threatening. I think that the government does if for one or more of the Medicare options, the Advantage or the drug program. It would be better if google or webmd did it than the government, perhaps, but I think the function is useful.
