Affordable public health care with it's limitations like waiting lines for everything and thus rationing of services opens up for better performing private services to compete.

Don't forget, that the public health care doesn't come for free here in Germany and that anybody earning 48k€+ can decide if he wants the fees to be added to his income taxes or rather diverted to a private insurance company of his choice.

My understanding on Obama's 'Rolls Royse for everybody' insurance is, that it would come for free and be paid by taxing the fat cats only, right?

If that would be possible, and I seriously doubt that, how could private enterprise compete with that???
